As the parent/guardian, you understand that the purpose of this Summer Camp for students ages 5-13 is to offer a diverse and engaging program that combines sports, language learning, cultural experiences, and social emotional learning. It is important that the students want to engage in this camp and that he/she is willing to participate in the simulated activities and Spanish. Parents will be notified if the student is creating an environment that does not allow the teacher and students to fulfill the daily objectives. In the event of this, the student will be asked not to return and the parent will not be refunded.
Miami Multilingual Sports Camp, LLC is not responsible for lost or broken items. Each camper is responsible for his/her own belongings. In the event of an emergency, by signing you authorize a camp director to administer to your child any treatment deemed necessary. You authorize a camp director to engage your child in a variety of learning activities (sports, excursions, music, film clips, arts, crafts, simulations, dance, exploration walks, food tasting *PLEASE FILL IN ALLERGIES WHERE SPECIFIED*), and any age appropriate activities planned in the scope and sequence created by Miami Multilingual Sports Camp, LLC.
I AGREE TO FULLY RELEASE, HOLD HARMLESS, AND DISCHARGE Miami Multilingual Sports Camp, LLC FROM ANY AND ALL LIABILITY, AND COVENANT NOT TO SUE MMSC for any damages, injuries, or claims, including any injuries or damages occurring from any negligence of MMSC to my child or any other person, as a result my or my child's participation in the ACTIVITIES. I fully understand and agree to release, discharge, hold harmless, and covenant not to sue MMSC, forever, for any alleged negligence in causing any such injuries or damage, in whole or in part, to myself, my property, or other person or property.
By signing this you are also hereby granting permission for Miami Multilingual Sports Camp, LLC to use and publish photographs and videos taken of your child, for promotional purposes on social media platforms, camp website, newsletters, and other marketing materials.
Due to the limited space available, once you have submitted your registration form and payment, your child's spot is reserved. Furthermore, once full payment is remitted, no refunds will be made.
For the safety of other camp attendees, please do not send your child if he/she is sick.
